RBMS Controlled Vocabularies Proposal Form

This form is to be used for proposing new terms to be added to any of the RBMS controlled vocabularies, or to propose changes to existing terms. For new terms, complete section 1 and as much of section 2 as possible--more complete information in section 2 will speed consideration of new terms. For changes to existing terms, complete as much as possible of section 3.

Use a separate form for each term. If possible, please indicate a cataloging record in which the term is or would be used, either by giving its bibliographic utility ID number or local system ID number with telnet or WWW address.
